What is the Single Integrated Plan?
The Single Integrated Plan is the document that will frame public, private and voluntary sector activity in Blaenau Gwent over the period 2013-2017. It describes the issues that are important to Blaenau Gwent and identifies the approach that a range of organisations will adopt in attempting to tackle them.
Who is the Single Integrated Plan for?
The Single Integrated Plan is for everyone who lives in, visits or has an interest in Blaenau Gwent. It has been prepared for a range of audiences; for Blaenau Gwent citizens, for the organisations that serve Blaenau Gwent and as a means of communicating our priorities to external agencies and the Welsh Government.
Who has prepared the Single Integrated Plan?
This document has been prepared by Blaenau Gwent's Local Service Board (LSB) and its associated partnerships. LSBs have been established in every local authority area in Wales with the purpose of providing collaborative leadership and to identify opportunities for service providers to work more closely together.
How has the Single Integrated Plan 'Making a difference' been prepared?
In preparing this document we have analysed data from a wide range of sources and brought that information together in a "Unified Needs Assessment". We have also reviewed other plans and strategies that are important to Blaenau Gwent.
The Plan builds on, and brings together in one document the joint work previously developed through the Community Strategy that we published in 2011, as well as the Community Safety, Children and Young People's and the Health Social Care and Well-being Partnerships' plans.
How will the Single Integrated Plan be implemented?
'Making a difference' will be implemented by Blaenau Gwent's new Strategic Partnership Framework which was agreed by the Local Service Board in May 2012. The framework will ensure that public services, and voluntary and community organisations serving the people of Blaenau Gwent work together to tackle what is really important to achieve improved outcomes.
From April 2013, Blaenau Gwent's new Strategic Partnership Framework will replace the existing Children and Young People Partnership; Health, Social Care and Well-Being Partnership, Community Safety Partnership and the Regeneration Partnership.
How will you know whether or not the Single Integrated Plan has been successful?
It is important that we are able to evaluate whether or not Blaenau Gwent's Single Integrated Plan has made a difference.
To show how the Local Service Board will achieve the intentions set out in this Single Integrated Plan, there is a time-aligned action plan for each of the priorities set out in the five themes (thriving, learning, healthy, fair and safe and vibrant).
Each action plan demonstrates partner organisations' commitment to delivering the Single Integrated Plan, which they will be held to account over, and scrutinised over the term of the plan. These action plans will run for a four year period from April 2013 to March 2017, being refreshed and updated annually.
The action plans are set out for each theme, under each priority as follows:
What we will be doing;
Who will make it happen;
How will we know we are on track;
What difference will it make
How to find out more; and
How actions have an impact across the themes.
To show how the Local Service Board and the Blaenau Gwent Strategic Partnership Framework is progressing towards achieving the intentions, quarterly performance monitoring will be carried out utilising a Result Based Accountability approach. This will include regular checking against the actions in the plan, and holding the appropriate people accountable for their contribution. Furthermore, on an annual basis a progress report will be produced, with the intention of making this readily available to key partners, stakeholders and citizens in appropriate formats.
The plan is due to come to an end and will be superseded by The Blaenau Gwent Well-being Plan in Spring 2018
